Want to start a club on campus?

  • New Club/Organization Charter Form

Need to request to host an event on and/or off campus? (Ex. Club meetings, Fundraiser at Restaurant, Volunteering at a dog shelter)

  • Event/Facility Request Form

Want to host a fundraiser?

  • Fundraising Proposal Request

For the day of the fundraising event, do not forget to fill out the Event/Facility Request Form.

Do you have fundraising money to deposit in the organization account?

  • Deposit Form

Need to report an activity or meeting the club has already done? (Ex. Club meetings, Fundraiser at Restaurant, Volunteering at a dog shelter)

  • Activity Reporting Form

Want food for a club meeting or event? (This comes out of your SLA allotted funds per semester unless specified)

  • Food Order Form

Traveling with the organization? (Ensure you talk with SLA staff before filling these forms out)

  • Release of Liability Form
  • Combined Travel Waiver and Transportation Election Form
